Job summary

Oak Street Health is seeking a Patient Growth Specialist (Outreach Executive I) in Phoenix to connect Medicare-eligible seniors with our care model. You will generate leads through community events and local outreach while acting as a brand ambassador for Oak Street Health.

You will collaborate with Outreach Managers and Directors to expand our patient base, manage relationships, and document activities in the CRM.

Qualifications

2+ years in Sales or Healthcare environments.
Fluent in English and Spanish.
Proven ability to cold call and manage digital lead channels.
Exceptional verbal and written communication skills.
Must have a valid driver’s license and reliable transportation; 60% field-based and 40% clinic-based.
Willingness to work weekends for community events and patient engagement.
US work authorization.

Responsibilities

Generate high-quality leads by executing community events and localized outreach within the Medicare senior community.
Identify and convert leads through cold calling, digital channels, and referrals from the broader outreach team.
Persistently engage prospective patients via email, phone, and face-to-face networking to build a robust sales pipeline.
Meticulously document all customer interactions and sales activities within the CRM and internal systems.
Partner with CRMs and Outreach leadership to align on lead generation strategies and IA partnerships.
Serve as the primary liaison between clinical teams and new patients to ensure a seamless transition into our care.
