Job summary

Matter Health is seeking a Patient Advocate & Liaison to work directly in community settings, building trusted relationships with older adults in public housing communities. You’ll educate patients about our care model, support enrollment, coordinate follow-ups, and help patients stay connected with needed services.

In this field-based role, you’ll collaborate with clinicians and operations teams to reduce barriers to care and improve access.

Qualifications

• 1–2 years of experience in community outreach, patient advocacy, healthcare, social services, customer service, or related roles.
• Excellent communication and relationship-building abilities.
• Comfortable working independently in field-based settings.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Build trusted relationships with older adults in assigned communities.
• Serve as primary representative for Matter Health in the community.
• Educate prospective patients about Matter Health’s care model and services.
• Support outreach, enrollment, scheduling, and ongoing engagement.
• Conduct in-person outreach, phone calls, and community events.
• Partner with APPs, Social Workers, Care Navigators, and Operations teams.
• Identify barriers to care and connect patients with resources.
• Document interactions in Salesforce/Athena and maintain records.
