Job summary

DESC in Seattle seeks an Outreach Nurse Supervisor to oversee RNs and Medical Assistants delivering nursing care through HOST, COAT and PACT teams, with at least 20% direct client care. The role emphasizes community-based services, housing-first principles, and collaboration with psychiatry to ensure continuity of care.

Responsibilities include supervising nursing staff, coordinating medication management, and providing patient education while working in home and community settings across DESC

Qualifications

• Registered Nurse licensed in Washington state.
• Experience with chronic mental illness and homelessness populations is preferred.
• Strong leadership and supervisory skills are needed.

Responsibilities

• Oversee PACT, HOST and COAT nurses and their activities within the team.
• Supervise nursing staff, schedule, and performance evaluations.
• Coordinate medication management, orders, and education for clients and staff.
• Collaborate with psychiatrists, primary care, and laboratories to ensure continuity of care.
• Provide on-call backup supervision to evening and weekend staff.
• Document progress and participate in treatment planning meetings.
