Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to provide patient-centered care in a field-based, full-time capacity. You will work with patients and families to restore function and independence in daily activities at home.

Responsibilities include conducting functional assessments, creating treatment plans, coordinating with interdisciplinary teams, and supervising COTAs. Required are an accredited OT program, NBCOT certification, state licensure, CPR, and a valid driver’s

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state OT licensure.
• Minimum of one year of clinical experience; home health preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status and independence at home.
• Assess home safety and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function and daily living skills.
• Fabricate splints and train use of adaptive devices.
• Supervise COTAs as per state regulations.
• Coordinate with care team and participate in discharge planning.
• Document care in the EMR.
