Job summary

ForzaCare, based in Waterloo, Iowa, is seeking a Field Medical Case Manager to help injured workers navigate their recovery and return to work. You’ll serve as the central point of coordination, connecting the worker, medical providers, employers, and insurance carriers to ensure timely, transparent care management.

This role is ideal for licensed nurses or certified rehabilitation counselors. Prior workers’ compensation experience is strongly preferred, and travel to appointments is required

Qualifications

• Active RN or CRC license with related degree.
• CCM, CIRS, or other case management credentials preferred.
• Willing to travel to appointments with valid driver’s license.
• Prior experience in workers’ compensation case management is strongly preferred.

Responsibilities

• Coordinate care between providers, employers, insurance carriers, and injured workers.
• Attend appointments at employer or worker locations as needed.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide progress reports to clients and stakeholders.
• Ensure case management meets customer and compliance requirements.
• Maintain relationships with clients, providers and team members.
