InclusivCare is seeking a Nurse Care Manager to oversee chronic care management, education, and care coordination for high-risk patients in an outpatient setting. The role requires strong leadership, clinical expertise in nursing, and collaboration with a multidisciplinary team to optimize patient outcomes within the community.

The Nurse Care Manager will educate patients and families, coordinate with care teams, and ensure documentation and care plans reflect patient needs.

Formación

• Experience in Case Management, Home Health, Medical/Surgical Nursing, Emergency Room or Ambulatory Care.
• Five years of nursing experience required.

Responsabilidades

• Provide timely, coordinated, interdisciplinary care in collaboration with the care team.
• Maintain EMR updates to optimize workflows and documentation.
• Educate patients and families about treatment plans and self-care.
• Coordinate planning sessions to engage all disciplines in managing high-risk patients.
• Identify high-risk patients and triage based on disease severity and social factors.
