Job summary

CareSource is seeking a Nurse Care Manager to provide care coordination, in-home assessment, planning, facilitation, advocacy and authorization of covered plan services to meet the member’s health needs while promoting quality, cost-effective outcomes.

The role requires assessing needs across the continuum, authorizing services, collaborating with members, families, PCPs and the IDT, performing home visits, and developing patient-centered service plans to support independent living.

Qualifications

• Associates degree in Nursing required.
• Bachelor’s degree in Nursing preferred.
• Two years of experience as a registered nurse required.
• Current NY RN licensure required; Case Management Certification preferred.

Responsibilities

• Assess and monitor members’ needs and status across the care continuum.
• Authorize covered services and coordinate care with the IDT.
• Collaborate with member, family, caregivers, and primary care practitioners.
• Conduct home visits to assess living situations and needs.
• Develop Patient Centered Service Plans and document care management per policy.
