Job summary

CareSource is seeking a Nurse Care Manager to provide comprehensive care coordination, including in-home assessments, planning, facilitation, advocacy and authorization of covered services to meet members’ health needs and promote quality, cost-effective outcomes.

The role involves collaboration with members, families, caregivers, primary care providers and the IDT to develop and implement the Patient Centered Service Plan and ensure safe discharge planning across care settings.

Qualifications

• Associates degree in Nursing required; Bachelor’s degree preferred.
• Two (2) years of RN experience required.
• Clinical experience in geriatrics or managed long-term care preferred.
• Experience with multiple languages may be required based on operational needs.

Responsibilities

• Coordinate care across hospital, sub-acute, long-term and home settings.
• Assess, plan and provide ongoing care management for members.
• Authorizes services and coordinates care regardless of payer.
• Work with member/family and IDT to develop Patient Centered Service Plans.
• Document care management activities per policy and plan requirements.
