Job summary

SCAN Health Insurance is seeking an Advanced Practice Clinician to provide and coordinate primary care for SCAN members, often in their residences within senior living communities or skilled nursing facilities. The role emphasizes preventive care, chronic disease management, and care coordination with a multidisciplinary team.

The position requires an NP or PA-C, Arizona licensure, and willingness to travel extensively.

Qualifications

• Master’s degree in Nurse Practitioner or PA program.
• Arizona NP license or PA-C in good standing.
• Wound care and/or palliative care certificates are a plus.
• Knowledge of complex chronic diseases (e.g., dementia, diabetes, heart failure, COPD).
• Knowledge of HCC and ICD-10 coding and documentation.
• Travel 75%+ in assigned area; valid driver's license and auto insurance required.

Responsibilities

• Provide and coordinate primary care in patients’ residences, often in senior living communities or skilled nursing facilities.
• Perform exams, vaccines, tests, and triage; coordinate care to avoid unnecessary hospital visits.
• Develop comprehensive care plans with patients, families, and living-community staff.
• Collaborate with case management to ensure timely access to services.
• Engage with physicians and other care team members to promote prevention and chronic disease management.
• Document visits per protocol and support member education and advocacy.

Education

Master’s degree in Nurse Practitioner or PA program

Arizona NP license or PA-C (in good standing)

Wound care certificate

Palliative care certificate

Knowledge of CMS guidelines and Medicare Advantage

HIPAA knowledge

Willingness for travel 75%+
