Job summary

180 Degrees operates Brittany’s Place Transitional Living Program in St. Paul and seeks a dedicated TLP Case Manager to support young women in gaining housing stability and independence.

You’ll work 1:1 with residents, assess goals, navigate systems, and promote healing with a trauma-responsive, youth-centered approach. Roles include evening/weekend coverage and strong documentation across HMIS.

Qualifications

• Bachelor’s degree in human services or related field and two years of case management experience.
• Two years’ nonprofit sector experience required.
• Trauma-informed and youth-centered practice with strong documentation skills.

Responsibilities

• What you’ll actually do: work 1:1 with residents to identify goals and move toward housing stability, employment, and financial wellness
• Help residents navigate systems and build independent living skills
• Use trauma-responsive, youth-driven approaches in every interaction
• Foster safety, belonging, and connection to support healing and growth
• Maintain documentation across multiple databases (HMIS) to meet funding requirements
• On-call rotation and flexible hours including evenings and weekends
• Provide back-up coverage for the other TLP site when needed
