Job summary

Well Care Services is seeking a full-time Mental Health Case Manager to provide field-based services to adults with serious mental illness in the Las Vegas area. You will assess clients' strengths and needs and develop individualized service plans to help them reach their recovery goals.

Responsibilities include completing service assessments, linking consumers with community resources, coordinating services, and monitoring goal attainment while collaborating with other professionals.

Qualifications

• Minimum of two years of experience working with adults with serious mental illness or equivalent training/experience.
• Experience with interview techniques, service planning, and consultation.
• Familiarity with electronic medical records and ability to type.
• Good driving record; must not have six or more negative points in past three years or major violation in past four years.

Responsibilities

• Assess clients' strengths and needs and develop individualized service plans.
• Complete services assessments and link consumers with community resources.
• Coordinate services including health care and monitor attainment of goals.
• Collaborate with other professionals and perform duties as required. Pass drug and background checks.
