Job summary

WellPower is seeking a Clinical Case Manager to provide comprehensive psychosocial case management within a continuum of services designed to support recovery. You will assess needs, monitor progress, arrange transportation, and document interactions to help people access basic resources and services.

Responsibilities include coordinating with teams, maintaining trauma-informed care, and ensuring proper documentation.

Qualifications

• Bachelor (CM I) or Master (CM II) degree in psychology, social work, or related human services field.
• One year experience in mental health or human services preferred.
• Must have valid Colorado Driver’s license and a recent clean driving record.

Responsibilities

• Help people we serve meet basic needs for food, clothing, shelter, safety, and access to benefits.
• Arrange transportation for people we serve or take them in your vehicle.
• Provide psychosocial services to improve or maintain people’s ability to function socially.
• Identify and engage natural support systems and community resources.
• Maintain a trauma-informed environment of wellbeing and complete clinical documentation.
