Job summary

UNLV Health is seeking a case management professional to assess clients’ needs, develop individualized treatment plans, and connect clients with community services. The role requires collaborating with clients to set measurable goals and to implement plans, with periodic review and documentation for clinical and billing purposes.

Candidates must be legally authorized to work in the United States; UNLV Health does not provide sponsorships.

Qualifications

• Bachelor’s degree in a health related field from an accredited four-year college or university.

Responsibilities

• Engages clients to assess needs and strengths, gathers data using intake packet, and obtains release to obtain records and information relevant to continuity of client care.
• Develops individualized client-centered treatment plan by reviewing assessment and client’s level of functioning.
• Assists clients in developing measurable goals and objectives.
• Assists the client in developing a plan to achieve and implement the objectives.
• Reviews the treatment plan with clients at maximum of 3-month intervals.
• Maintains electronic case file and documents implementation of plans for clinical and billing purposes.
