Enki Health Services, Inc. is seeking a Mental Health Worker/Case Manager to support treatment planning and client recovery in community settings.

The role focuses on developing and implementing treatment plans to promote stability and higher functioning for clients. Responsibilities include monitoring adherence to plans, coordinating with local agencies, maintaining client records, and facilitating group and support activities.

Qualifikationen

• High school diploma plus two years' experience in a mental health setting, or a Bachelor's degree in a related field.
• Ability to develop individualized treatment plans with clients.
• Strong communication and documentation skills.

Aufgaben

• Monitor client compliance with treatment as prescribed by the treatment team.
• Develop individualized case management treatment plans in the Full Service Partnership (FSP) Program.
• Assist with establishing coordination with local agencies in the provision of services.
• Assist with the development and maintain resource directory for agency.
• Update and maintain client records.
• Arrange for medical care-exams, emergency shelter, food and/or education services for clients at large.
• Facilitate support/self-help groups for clients, family members and the community.
• Participate in case disposition meetings on a routine basis with the assigned treatment team.
• Provide and document services in accordance with the Rehabilitation Option/Targeted Case Management requirements.
• Participate in the Mental Health education program provided to the community.
• Coordinate client care with other providers both within and outside the agency.
• Attend and participate in all in-service training as necessary.
• Responsible for timely completion of documentation in the client’s clinical record per Enki, LA County Department of Mental Health and legal requirements.
• Responsible for meeting assigned minimum productivity requirements on a monthly basis.
• Other duties as assigned.
• Comply with all Company policies and procedures as applicable.
