Centers for Independence in Wisconsin is seeking a Case Manager to deliver care and treatment to adults with severe mental illness. You will develop and implement individualized treatment plans, conduct follow-up, and coordinate with a multidisciplinary team to ensure quality care.

The role requires a Bachelor’s degree in Social Work or related field, at least one year of experience, and strong communication skills in English and Spanish.

Qualifications

• Bachelor's Degree in Social Work, Counseling or related field.
• Minimum 1 year of experience with adults with severe mental illness.
• Ability to communicate clearly in English and Spanish (oral and written).
• Proficiency with Word and Excel; reliable and punctual.
• Ability to assist with activities of daily living; strong interpersonal skills.

Responsabilités

• Develop a treatment relationship with a client and formulate treatment plans on his/her caseload that will detail treatment to be provided.
• Provide psychosocial rehabilitation, symptom monitoring & management and supportive counseling to clients on caseload and participate in team case management.
• Monitor client's psychiatric medication regime and compliance as directed by the physician.
• Provide crisis intervention and supportive counseling to clients who are in crisis.
• Coordinate utilization of various community resources, day programs and outside health care providers.
• Assist clients with housing and income resource development; coordinate access to entitlement programs.
