Enki Health Services, Inc. seeks a Mental Health Worker/Case Manager to support treatment planning and community placement for clients with housing needs.

The role emphasizes recovery, service coordination, and treatment plan development to improve functioning in daily living. Responsibilities include documenting care, coordinating with local agencies, and ensuring timely completion of records in compliance with Enki and LA County DMH requirements.

Formación

• Two years of mental health experience or a Bachelor's degree.
• Valid California driver's license and automobile insurance.
• Bilingual English/Spanish is a plus.
• Proficient with MS Office and EMR systems.

Responsabilidades

• Monitor client compliance with treatment as prescribed by the treatment team.
• Develop individualized case management treatment plans with the client.
• Assist with establishing coordination with local agencies for services.
• Update and maintain client records and resource directories.
• Arrange for medical care exams, shelter, food and education services as needed.
• Facilitate support/self-help groups for clients and families.
