Responsibilities

• Assists in developing and coordinating holistic care management activities.
• Evaluates member needs and resources to plan the best outcome.
• Coordinates between members, families, and care providers to ensure services are accessible.
• Provides education to members and families on services, benefits, and options.
• Monitors care plans and outcomes; reports recommendations for improvement.
• Maintains member information to comply with state, federal, and payer regulations.
