Centene is seeking a field-based Care Manager in New York to assess and coordinate care for high-acuity members enrolled in MLTC programs. The role involves developing personalized care/service plans, educating families, and coordinating with providers to ensure quality, cost-effective care.

Qualified applicants will have RN or NP licensure and a nursing degree, along with 4–6 years of related experience. The position supports collaborative teams and may involve home visits as needed.

Formación

• Requires RN licensure or NP licensure with current state credentials.
• Bachelor's in Nursing preferred; advanced nursing licensure may be required.
• Experience in care management and long-term care settings is desirable.

Responsabilidades

• Assess and coordinate medical and supporting services for high acuity members.
• Develop and continuously assess long-term care/service plans with the care management team.
• Coordinate between member/family and care providers to ensure person-centered care.
• Monitor progress toward goals and revise care plans as needed.
• Educate members and families about disease processes, services, and benefits.
• Maintain member information to ensure compliance with guidelines.
