Job summary

Elevance Health is seeking an LTSS Service Coordinator - RN Clinician for field-based work across IN counties. The role includes managing member care plans, coordinating with the health team, and performing telephonic or in-person assessments to optimize care across the continuum.

This position requires an active RN license, 3+ years of relevant experience, and travel to client sites. Hybrid work is described; regular onsite presence may be required per policy.

Qualifications

• HS diploma or equivalent.
• 3+ years of experience with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active RN license in applicable state(s) required.
• State-specific certification may be required based on state law and/or contract.
• Associates/MA/MS in Health/Nursing preferred.
• Travel to worksite and other locations as necessary.

Responsibilities

• Manage member cases within licensure scope and develop care plans.
• Provide direction to non‑RN clinicians participating in the member's care.
• Perform telephonic or face‑to‑face assessments to identify, evaluate, and coordinate member needs.
• Coordinate services within benefits structure or through arrangements as permissible.
