Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments and coordinate LTSS/BH/PH needs with the care team.
• Identify members with potential clinical needs and coordinate cases with clinical management.
• Manage non-clinical needs of members to ensure efficient LTSS utilization.
• Document short- and long-term goals with the member and care team.
• Identify members who may require an alternative level of service.
• Mentor or preceptor for new staff and support process improvements.
• Submit utilization/authorization requests aligned with the care plan.
• Report critical incidents to appropriate state and county agencies.
• Assist with appeals, grievances, and state audits.
