Qualifications

• Georgia LPN license: must possess a currently active and valid Georgia license.
• Clinical proficiency in patient assessment, medical terminology, and safe medication administration.
• Experience in long-term care, geriatrics, or in-home care is preferred.
• Strong self-management and time-management without direct daily oversight.
• Valid driver’s license and dependable vehicle; mileage reimbursed.
• Professional, compassionate communication with clients and families.
• Background check clearance required.

Responsibilities

• Field supervision: bi-monthly in-home visits to evaluate client well-being and caregiver performance.
• Calendar autonomy: manage your own schedule by coordinating assessment and visit times with clients and families.
• Digital records: ensure all clinical documentation and progress notes are accurately maintained in digital systems.
• Collaborative planning: work with families and the clinical team to update care plans based on client health changes.
• Professional liaison: maintain proactive communication with community partners and families regarding client status or updates.
• Regulatory adherence: uphold HIPAA compliance and participate in internal record audits.
• Payroll reporting: record mileage, visit counts, and administrative hours for payroll processing.
