Job summary

Bay Area Community Services seeks a Clinical Care Coordinator to deliver comprehensive therapeutic interventions and case management in San Mateo. The role supports wellness, recovery, housing navigation, crisis intervention, and family engagement, with after-hours coverage as needed.

In this FSP program, you will work within an ACT framework across in-office and community settings, collaborating with teams to stabilize clients and connect them to housing and services.

Qualifications

• MSW, MFT, or MPCC required.
• Associate ASW/LCSW or Associate MFT/LPCC with BBS registration.
• 2 years of social service experience required.
• Bilingual in a threshold language preferred.
• California driver’s license and vehicle; travel required.

Responsibilities

• Develop and deliver a full complement of services (individual, family, group counseling, rehab, case management).
• Use Evidence-Based Practice to ensure high-quality results.
• Assess risk status and intervene appropriately.
• Create client-centered service plans to improve well‑being.
• Engage clients and report disengagement per protocols.
• Coordinate care planning, services, and benefits.
• Maintain data, reporting, and charting per policies.
• Participate in supervision, meetings, and training.
• Perform other duties as assigned.
