Job summary

Bay Area Community Services is seeking a Care Coordinator- Outreach to deliver therapeutic interventions and case management for individuals facing homelessness in Alameda County. The role involves field-based travel to Oakland, Berkeley, Fremont, and surrounding areas, providing direct, face-to-face services and coordinating care.

The position requires a high school diploma with at least four years in mental health/social services, a valid CA driver's license, and willingness to travel locally.

Qualifications

• High School Diploma required and 4 years of paid work experience in mental health/social services; degree preferred.
• Valid California driver’s license, personal vehicle, and proof of insurance; DMV printout required.
• Extensive local travel is required.

Responsibilities

• Develop and deliver a full complement of services, including individual, family, group counseling, rehabilitation and case management for clients.
• Utilize evidence-based practice to assure high quality results and impact.
• Continuously assess risk and intervene as needed based on symptoms and environment.
• Create person-centered service plans to help clients improve and achieve goals.
• Engage in outreach and manage disengagement according to program protocols.
• Coordinate care planning and documentation to comply with policies.
