Job summary

Child & Family Services, Inc. in Fall River, MA is seeking a Family-Based Intensive Treatment Paraprofessional to join a multidisciplinary team.

You will support intensive family therapy, care coordination, and targeted case management for youth under 21 with Serious Emotional Disturbance, under clinical supervision. You will assist clinicians, participate in family therapy, coordinate with schools and outpatient providers, and contribute to discharge planning and ongoing program supervision.

Responsibilities

• Collaborate with the team and family to implement interventions and strengthen family dynamics.
• Maintain positive relationships with FIT team, supervisors, clinicians, and community providers.
• Support clinicians' treatment plans to help youth and families achieve goals.
• Provide coaching in decision-making during crises and non-crisis periods.
• Attend and assist with family therapy sessions and facilitate individual sessions.
• Assist in implementing therapeutic objectives addressing youth mental health and behavior.
• Make referrals with the family and FIT team to access services.
• Link youth and families to outpatient providers and attend related meetings when needed.
• Assist families with education system needs; attend school meetings and IEP discussions.
• Identify and obtain services from available resources per the treatment plan.
• Attend meetings related to youth's behavioral health needs and support discharge planning.
• Participate in supervision and team meetings; engage in ongoing program activities.
• Engage in care coordination and collaboration across systems.
