Job summary

UnitedHealthcare is recruiting an Integrated Care Coordinator to serve as the primary care manager for a panel of older adult members with complex medical and behavioral health needs. You will coordinate medical, behavioral and social needs to promote appropriate service use and better quality of care.

The role requires travel up to 75% within the assigned Indiana territory and offers a Monday–Friday daytime schedule.

Qualifications

• Must possess a Master’s in Social Work or a Bachelor’s in Social Work, Psychology, Special Education, Counseling or related field with 1+ years of case management experience with older adults.
• 2+ years of experience in long-term support services or working with older adults.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Ability to travel 75% of the time within the assigned territory.
• Reside in Indiana and have reliable transportation & a valid US driver’s license.

Responsibilities

• Serve as primary care manager and service coordinator for high medical risk members with comorbid behavioral health needs.
• Engage members face-to-face or telephonically to complete comprehensive needs assessments across medical, behavioral, functional, cultural and socio-economic dimensions.
• Develop and implement individualized, person-centered care plans with goals and interventions.
• Partner with internal care team, providers and community resources to implement care plans.
• Provide referrals and linkage to appropriate services and programs as needed.
