Job summary

UnitedHealth Group seeks an Integrated Care Coordinator in Indiana to manage a panel of older adults with medical and behavioral health needs. You will assess, plan, and coordinate services across medical, behavioral health, and social determinants of health to support optimal outcomes.

Your role involves collaboration with care teams, providers, and community partners, plus travel within Vanderburgh County and surrounding areas. Telecommute options may apply as needed.

Qualifications

• Master's Degree in Social Work or Bachelor's in SW/Psychology/related field with 1+ year case management experience for older adults.
• 2+ years of experience in long-term support services or working with older adults.
• 1+ years of experience with local behavioral health providers addressing SDoH (e.g., housing, transportation, food access).
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Ability to travel 75% within the assigned territory.
• Reside in Indiana and have reliable transportation + valid US driver’s license.

Responsibilities

• Serve as primary care manager and service coordinator for high medical risk/needs members with behavioral health needs.
• Conduct comprehensive needs assessments across medical, behavioral, functional, cultural, and socioeconomic domains.
• Develop and implement person-centered care plans aligned with readiness to change to improve health outcomes.
• Collaborate with internal care teams, providers, and community resources to execute care plans.
• Provide referrals and linkage to internal/external services as appropriate.
