Job summary

Pacific Health Group in Siskiyou County is seeking a field-based Housing Stability Services Specialist to support Housing Transition Navigation, tenancy & sustaining services, and deposit assistance.

You will conduct assessments, develop care plans, coordinate housing resources, and build partnerships with providers while documenting in state-compliant charts. This role requires independence, strong time management, and the ability to work in the community.

Qualifications

• Strong understanding of housing systems and local housing resources.
• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Strong understanding of healthcare systems, managed care, and community resources.
• Excellent communication, empathy, and cultural competence.
• Strong organizational and time management skills.
• Proficient with high-volume documentation and case management systems.

Responsibilities

• Provide Housing Transition Navigation Services to move members into stable housing.
• Coordinate Housing Tenancy & Sustaining Services for ongoing housing stability.
• Assist with Housing Deposits, including documentation and deposits access.
• Collaborate with housing authorities, landlords, shelters, and providers to secure placements.
• Monitor housing stability and address barriers impacting health outcomes.
• Conduct field visits, assessments, and follow-ups across county areas.
