Amedisys Home Health seeks a Hospice Liaison for the Atlanta and Alpharetta area. This outside sales role focuses on educating providers, patients, and families about home care services, while developing key referral relationships to drive admissions and growth.

Qualifications

• At least one year of experience in a healthcare-related business development/outreach role.
• Bachelor's degree preferred, but not required.

Responsibilities

• Educates healthcare professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources (hospitals, nursing homes, physician offices) to generate leads.
• Coordinates patient admissions with the team, collecting required documentation for referral to admission.
• Conducts on-site visits to assess patient needs and barriers to care.
• Keeps accurate records of patient interactions and referrals.
• Achieves admissions, product, and payor-mix goals; meets short and long-term target account goals.
• Documents daily sales calls in CRM with pre/post-call notes.
• Maintains up-to-date records of targeted referral sources in territory in CRM.
• Attends community events to promote Amedisys and responds to customer inquiries promptly.
