Job summary

Amedisys is seeking a Hospice Liaison in the Virginia region to drive outside sales and business development within the home health and hospice space. You will educate clinicians, patients, and families about home care services and build key referral partnerships to grow admissions.

The role involves field travel (50% or more) within the assigned territory, with competitive pay and monthly incentives. A Bachelor’s degree is preferred, along with healthcare outreach experience and reliable

Qualifications

• At least one year of experience in a healthcare related business development/outreach role.
• Bachelor's degree preferred but not required.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources (hospitals, nursing homes, physician offices) to generate leads.
• Coordinates patient admissions with the team and secures required documentation for referral to admission.
• Conducts on-site visits to assess patient needs and identify barriers to care.
• Keeps accurate records of patient interactions and referrals in CRM.
• Meets admissions, product, and payor-mix goals and targets.
• Documents minimum daily sales calls in CRM with pre/post call notes.
• Maintains a complete record of targeted referral sources in territory in CRM.
• Attends community events to promote Amedisys and respond to inquiries.
• Performs other duties as assigned.
