Job summary

Amedisys is seeking a Hospice Liaison to drive outside sales and develop relationships with hospitals, nursing homes, and physician offices within Forsyth County. This role involves educating care teams about home health and hospice services, coordinating admissions, and meeting growth targets.

Travel of 50%+ within the territory is required. Competitive base salary with monthly incentives, and comprehensive benefits are offered.

Qualifications

• 1+ year in healthcare-related business development/outreach.
• Bachelor's degree preferred but not required.
• Reliable transportation and valid driver's license.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team and obtains required documentation.
• Performs on-site visits to assess patient needs and barriers to care.
• Keeps accurate records of patient interactions and referrals in CRM.
• Meets admissions, product, and payor-mix goals; targets account goals.
