Job summary

Amedisys Home Health is seeking a Hospice Liaison to educate clinicians, patients and families about home care services and to build strong referral networks. The role covers Forsyth County with substantial travel, and requires proactive outreach and relationship building to achieve admissions and revenue goals.

Qualifications

• At least one year in healthcare-related business development/outreach.
• Bachelor's degree preferred but not required.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with referral sources to generate leads.
• Coordinates patient admissions with the team and completes referral documentation.
• Conducts on-site visits to assess patient needs and barriers to care.
• Keeps accurate records of patient interactions and referrals.
• Meets admissions, product, and payor-mix goals.
• Documents daily sales calls in CRM with pre/post notes.
• Attends community events to promote Amedisys.
• Responds to customer complaints promptly.
• Performs other duties as assigned.
