Job summary

Quality Home Health, Hospice, and Palliative Care is seeking a compassionate Hospice RN Case Manager in Utah County. The role involves coordinating patient-centered hospice care, conducting skilled nursing visits in patients' homes, and collaborating with an interdisciplinary team to ensure high-quality end-of-life care.

You will develop individualized care plans, manage a caseload, educate families, and provide emotional support while traveling throughout Utah County.

Qualifications

• Current, active RN license in Utah.
• Experience in hospice, home health, or palliative care preferred.
• Strong assessment, symptom management, and communication skills.

Responsibilities

• Provide skilled nursing assessments in patients' homes and other settings.
• Develop, implement, and update plans of care.
• Coordinate care for an assigned hospice caseload with the interdisciplinary team.
• Educate patients and families on disease progression and symptom management.
• Deliver compassionate emotional support to patients and families.
