Superior Home Care and Hospice in Salt Lake City, UT is seeking a qualified Hospice and Home Health RN Case Manager who shares our commitment to patient-focused care and long-term service.

Responsibilities include skilled nursing care, care coordination with physicians, goal setting with patients and families, chart compliance, and development of treatment plans. Utah RN license and CPR required; 1 year nursing experience preferred in hospice/home health.

Qualifications

• Licensed as a Registered Nurse by the State of Utah.
• CPR certification.
• A minimum of 1 year nursing experience is required.
• Hospice and home health experience preferred but not required.

Responsibilities

• Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.
• Create patient centered goals with patients, family members and providers.
• Assess, supervise and direct patient care.
• Develop plans of treatment.
• Ensure chart compliance with state regulations.
• Coordinate patient care with physicians and other providers.
