Quality Home Health, Hospice, and Palliative Care is seeking a compassionate Hospice RN Case Manager in Salt Lake County. The role involves coordinating and delivering patient-centered hospice care in patients' homes and other settings, with a focus on comfort and dignity.

You will manage a caseload, develop care plans, perform skilled nursing assessments, and collaborate with an interdisciplinary team to ensure high-quality care for patients and families.

Qualificações

• Active Utah RN license.
• Hospice, home health, or palliative care experience preferred.
• Strong assessment and communication skills.
• Willingness to travel within Salt Lake County.

Responsabilidades

• Provide skilled nursing assessments and hospice visits in patients' homes and other care settings.
• Develop, implement, and update individualized plans of care.
• Manage and coordinate care for an assigned hospice caseload.
• Assess and manage pain, symptoms, and changes in patient condition.
