Quality Home Health, Hospice & Palliative Care in Salt Lake County is seeking a compassionate Hospice RN Case Manager to coordinate patient-centered hospice care in patients' homes and local facilities. This field-based role focuses on building trusted relationships with patients and families while collaborating with an interdisciplinary team.

The ideal candidate will have an active Utah RN license, experience in hospice or related care, strong assessment skills, and comfort with EMR and travel

Qualifications

• Current, active Registered Nurse (RN) license in Utah.
• Previous hospice, home health, palliative care, or related nursing experience preferred.
• Strong clinical assessment, symptom management, and critical-thinking skills.
• Compassionate communication and a patient-centered approach to care.
• Ability to work independently in a field-based environment while collaborating with a supportive team.
• Strong organizational and time-management skills.
• Comfortable using electronic medical records and mobile technology.
• Valid driver's license, reliable transportation, and ability to travel throughout Salt Lake County.
• Current CPR/BLS certification, as required by agency policy.

Responsibilities

• Provide skilled nursing assessments and ongoing hospice visits in patients' homes and other care settings.
• Develop, implement, and update individualized plans of care.
• Manage and coordinate care for an assigned caseload of hospice patients.
• Assess and manage pain, symptoms, and changes in patient condition.
• Administer medications and provide nursing interventions within the RN scope of practice.
• Educate patients, families, and caregivers on disease progression, medications, symptom management, and comfort measures.
• Provide compassionate emotional support to patients and their loved ones.
• Recognize changes in patient needs and communicate promptly with the appropriate care team members.
