Home Caregivers Partnership LLC in Utah seeks a Hospice Case Manager RN to coordinate and provide comprehensive nursing care for hospice patients and families under the supervision of the Clinical Manager.

You will assess needs, manage symptoms, coordinate interdisciplinary services, educate and support families, and ensure timely clinical documentation to promote comfort, dignity, and quality of life throughout the hospice experience.

Qualifikationen

• Must hold a valid, unrestricted Utah RN license.
• Hospice, home health, oncology, geriatrics, or end-of-life care experience preferred.
• Ability to exercise initiative and independent judgment.
• Valid driver's license and reliable transportation.

Aufgaben

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and develops the plan of care with IDT.
• Reevaluates nursing needs and revises the care plan.
• Initiates plan of care and necessary revisions.
• Prepares clinical and progress notes for patient visits.
• Coordinates services with IDT and physicians.
• Informs physician of changes in condition.
• Counsels patient and family about nursing needs.
• Participates in peer review and quality management as assigned.
• Completes OASIS assessments and discharge summaries.
