Job summary

Quality Home Health, Hospice & Palliative Care is seeking a compassionate Hospice RN Case Manager to join our Utah County team. This field-based role focuses on delivering patient-centered hospice care in patients' homes, coordinating with an interdisciplinary team to support families and ensure comfort and dignity.

You'll develop individualized care plans, perform skilled nursing assessments, manage a caseload, educate families, and advocate for high-quality end-of-life care across Utah County.

Qualifications

• Current, active RN license in Utah.
• Hospice, home health, or palliative care nursing experience preferred.
• Strong clinical assessment and symptom management skills.
• Compassionate communication and patient-centered approach.
• Ability to work independently in a field-based environment.
• Excellent organizational and time-management skills.
• Comfortable using EMR and mobile technology.
• Valid driver’s license, reliable transportation, travel across Utah County.
• Current CPR/BLS certification as required by policy.

Responsibilities

Position Overview:

The Hospice RN Case Manager is responsible for coordinating and providing compassionate, patient-centered hospice care to patients and families throughout Utah County.

In this role, you'll manage a caseload of hospice patients, develop individualized care plans, provide skilled nursing assessments and interventions, and work closely with our interdisciplinary team to ensure patients receive the highest quality of care.

Our goal is to help patients experience comfort, dignity, and quality of life while providing families with the education, guidance, and emotional support they need.

Key Responsibilities:

Patient Care & Case Management:

• Provide skilled nursing assessments and ongoing hospice visits in patients' homes and other care settings.
• Develop, implement, and update individualized plans of care.
• Manage and coordinate care for an assigned caseload of hospice patients.
• Assess and manage pain, symptoms, and changes in patient condition.
• Administer medications and provide nursing interventions within the RN scope of practice.
• Educate patients, families, and caregivers on disease progression, medications, symptom management, and comfort measures.
• Provide compassionate emotional support to patients and their loved ones.
• Recognize changes in patient needs and communicate promptly with the appropriate care team members.

Care Coordination & Communication:

• Collaborate with physicians, hospice aides, social workers, chaplains, and other members of the interdisciplinary team.
• Participate in interdisciplinary group meetings and care planning.
• Coordinate necessary supplies, equipment, medications, and services.
• Communicate effectively with patients, families, caregivers, and facility staff.
• Help ensure smooth transitions of care and continuity of services.

Documentation & Compliance:

• Complete accurate and timely clinical documentation in the electronic medical record (EMR).
• Maintain documentation in accordance with hospice regulations, agency policies, and professional nursing standards.
• Participate in ongoing care planning, recertification assessments, and quality improvement efforts.
• Maintain patient confidentiality and comply with HIPAA requirements.

Qualifications:

• Current, active Registered Nurse (RN) license in Utah.
• Previous hospice, home health, palliative care, or related nursing experience preferred.
• Strong clinical assessment, symptom management, and critical-thinking skills.
• Compassionate communication and a patient-centered approach to care.
• Ability to work independently in a field-based environment while collaborating with a supportive team.
• Strong organizational and time-management skills.
• Comfortable using electronic medical records and mobile technology.
• Valid driver's license, reliable transportation, and ability to travel throughout Utah County.
• Current CPR/BLS certification, as required by agency policy.
