Superior Home Care and Hospice in Salt Lake City, UT is hiring a qualified RN Case Manager to deliver home health and hospice nursing care. You will create patient-centered goals, coordinate with physicians, and ensure chart compliance with state rules.

Qualificações

Licensed as a Registered Nurse by the State of Utah.
Current Utah driver’s license and automobile insurance.
CPR certification.
Minimum of 1 year nursing experience required.
Hospice and home health experience preferred but not required.
Education: Associate degree.
Experience: Nursing: 1 year.

Responsabilidades

Provide skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication teaching and hospice assessments.
Create patient-centered goals with patients, families, and providers.
Assess, supervise and direct patient care.
Develop plans of treatment.
Ensure chart compliance with state regulations.
Coordinate patient care with physicians and other providers.
