American RENOLIT Corporation is seeking a Registered Nurse (RN) Case Manager for hospice care in Indiana. You will care for patients in their homes, build meaningful relationships with families, and use clinical judgment to promote comfort and quality of life.

This exempt, field-based role offers autonomy, independent patient management, and collaboration with an interdisciplinary hospice team. You’ll mentor teammates and support patients throughout the end-of-life journey.

Qualifications

• Current Indiana RN license.
• Michigan RN license is a plus.
• Strong clinical assessment, communication and critical-thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Knowledge of hospice philosophy, coverage requirements, and state/federal regulations.
• Ability to meet background screening, TB testing and employment eligibility requirements.

Responsabilités

• Assess patient physical, psychosocial, environmental, safety and spiritual needs.
• Develop and manage individualized plans of care based on goals.
• Assess and manage pain, symptoms and comfort needs.
• Provide skilled nursing care in the patient’s residence.
• Educate and support patients, families and caregivers as needs change.
• Help patients and families understand medications and the plan of care and end-of-life journey.
• Coordinate care and communicate changes with physicians and the team.
• Participate in interdisciplinary group meetings to support coordinated, consistent care.
• Supervise LPNs and hospice/home health aides per requirements.
• Complete admissions as assigned.
• Maintain accurate, timely clinical documentation.
• Serve as a resource and preceptor for team members and students.
