Job summary

Heart to Heart Hospice in Indiana is seeking an RN Case Manager to join our compassionate care team. You will assess patients, develop individualized plans, and coordinate with IDG members to support comfort and quality of life.

Ideal applicants are licensed RNs with at least two years of clinical experience; a valid driver’s license and reliable transportation are required. We offer competitive pay and comprehensive benefits.

Qualifications

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Current State Registered Nurse License.
Current Driver's License and reliable transportation.
Two years of RN experience preferred in healthcare/clinical setting.

Responsibilities

Initial and ongoing comprehensive assessments of the impact of terminal disease on patient needs and ADLs.
Implement the individualized Plan of Care and revise as necessary.
Collaborates with patient, caregiver, physicians and healthcare professionals.
Attend Inter-Disciplinary Group (IDG) meetings and coordinate patient care planning.
Perform on-call responsibilities and provide on-call service as assigned.
Timely submission of all documentation.
Ensure compliance with laws, Medicare regulations and policies.
