Job summary

Center4Hospice is seeking a Registered Nurse (RN) Case Manager for Plymouth, Indiana. This full-time, day-shift position involves delivering hospice care in patients’ homes, coordinating with a skilled interdisciplinary team, and guiding families through end-of-life planning.

You’ll work weekdays with on-call rotation, enjoy autonomy in patient care, and build meaningful relationships while ensuring comfort and dignity for patients and families.

Qualifications

• Strong clinical assessment and critical-thinking skills.
• Excellent communication with patients, families and team members.
• Ability to work independently while coordinating care with an interdisciplinary hospice team.
• Knowledge of hospice philosophy, coverage requirements, and state/federal regulations is preferred.

Responsibilities

• Assess patients’ physical, psychosocial, environmental, safety and spiritual needs.
• Develop and manage individualized plans of care with patients and families.
• Assess and manage pain, symptoms and comfort.
• Provide skilled nursing care in the patient’s residence.
• Educate and support patients, families and caregivers as needs change.
• Coordinate care and communicate changes with physicians and the team.
• Supervise LPNs and hospice/home health aides per requirements.
• Maintain timely clinical documentation.
