Heart to Heart Hospice is seeking a dedicated RN Case Manager in Indiana to lead comprehensive assessments of patients facing terminal illness and coordinate their care with an interdisciplinary team. The role requires an active RN license, a nursing degree, and driving capability for community-based visits.

You will manage the care plan, attend IDG meetings, and provide on-call support as needed. Responsibilities include timely documentation, collaboration with families and healthcare

Competenze

• Must be a graduate of an accredited nursing program (Diploma, ADN, or BSN).
• Current state RN license in good standing.
• Two years of RN experience in healthcare/clinical setting preferred.

Mansioni

• Perform initial and ongoing comprehensive assessments of patients with terminal illness and their needs across physical, psychosocial, environmental and ADL aspects.
• Implement individualized plan of care (POC) and revise as necessary.
• Collaborate with patient, caregiver, physicians and care team.
• Attend IDG meetings and coordinate patient care planning with prioritization of needs.
• Provide on-call coverage and respond to on-call requests for patients/families.
• Ensure timely submission of documentation.
• Perform other duties as required and ensure compliance with laws and policies.
