American RENOLIT Corporation is seeking an experienced RN Case Manager for hospice care in Indiana. You’ll work in patients’ homes, manage care plans, and collaborate with an interdisciplinary team to support comfort and dignity at end‑of‑life.

You’ll assess needs, coordinate care, and educate families while maintaining documentation and adherence to regulations. A structured onboarding program supports new hospice nurses.

Competenze

• Current Indiana RN license is required.
• Michigan RN license is a plus.
• Ability to work independently while collaborating with an interdisciplinary team.
• Strong clinical assessment, communication and critical-thinking skills.
• Ability to meet background screening, TB testing and employment eligibility requirements.

Mansioni

• As RN Case Manager, coordinate care with the hospice team and serve as a trusted clinical resource for patients and families.
• Assess physical, psychosocial, environmental, safety and spiritual needs; develop plans of care.
• Manage pain, symptoms and comfort; educate and support patients and caregivers.
• Coordinate care and communicate changes with physicians and team; supervise LPNs and aides.
• Complete admissions and maintain accurate clinical documentation.
