Job summary

Tom Trese Networking LLC seeks an experienced Home Health RN Case Manager for field-based service in the Dallas/Fort Worth area. Responsibilities include skilled nursing visits, OASIS documentation, care plans with physicians and families, and coordination with the care team.

Local travel is required with reliable transportation. Candidates must hold an active Texas RN license and have recent Medicare home health case management experience.

Qualifications

• Active Texas RN license
• Recent home health case management experience
• Strong assessment and documentation skills
• Ability to travel locally throughout Fort Worth / Tarrant County
• EMR experience preferred

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the care team.
• Provide patient and caregiver education on disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
