Job summary

Aspire Talent Solutions is seeking an experienced Home Health RN Case Manager for a field-based role in the Dallas–Fort Worth area. You will manage a patient caseload in the home, perform skilled visits, and coordinate plans of care with the care team.

The role requires a Texas RN license and recent home health case management experience. Travel locally is required; EMR experience preferred. This PRN position supports Medicare-certified home health services.

Qualifications

• Active Texas RN license in good standing.
• Recent home health RN case management experience.
• Knowledge of Medicare home health, OASIS, and plan-of-care management.
• Ability to work independently with strong communication.
• Willingness to travel locally in Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
