Your Talent Team is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position requires an active Texas RN license and recent home health case management experience, with the ability to manage a caseload in the home setting.

Travel locally is required; documentation accuracy and strong communication with the clinical team are essential. EMR experience is preferred and the role is PRN in schedule.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management.

Responsabilités

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate plans of care with physicians, patients, families, and the care team.
• Complete accurate clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
