ExcelliSearch, Inc. is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas area in Fort Worth and surrounding counties.

The RN will manage a caseload in the home health setting, perform skilled visits, complete OASIS documentation, and coordinate care with physicians, patients, families, and the care team. Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver's license and reliable transportation; local travel throughout Fort Worth/Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate plans of care with physicians, patients, families, and the care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
• Educate patients and caregivers on disease management, medications, safety, and goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
