Job summary

Top Echelon Software, LLC seeks an experienced Home Health RN Case Manager for Fort Worth, TX and the surrounding Tarrant County area. This field-based role requires clinical excellence, strong assessment skills, and the ability to manage an assigned patient caseload independently in the home setting.

You will perform skilled nursing visits, assessments, recertifications, and care plan development in collaboration with physicians, patients, families, and the interdisciplinary team.

Qualifications

• Active Texas RN license in good standing is required.
• Recent Home Health RN Case Management experience is required.
• Strong assessment, documentation, and time-management skills.
• Ability to work independently in the field and collaborate with the care team.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the care team.
• Complete accurate clinical documentation including OASIS and visit notes.
• Monitor patient progress and communicate updates to the care team.
