Perfect Care Match is seeking a Community Relations Case Manager – Healthcare Sales & Growth for the Greater Atlanta, GA area. This field-based role focuses on building relationships, developing business, and driving measurable referral growth with uncapped commissions and incentives.

The position targets expanding our presence in Greater Atlanta with a competitive base salary of $68,000–$78,000 per year plus commissions, incentives, and bonuses. On-the-road work is a core expectation.

Qualifications

• Ambitious, entrepreneurial, and highly relationship-driven.
• Experience in community relations sales in healthcare or growth roles is preferred.
• Strong ability to build referrals and develop partnerships within healthcare networks.

Responsibilities

• Build referral relationships with hospitals, rehab centers, and physicians.
• Prospect and open new referral opportunities throughout Greater Atlanta, GA.
• Conduct in-person meetings, presentations, networking, and community outreach.
• Follow up on referrals and coordinate with care, clinical, recruiting, scheduling, and operations teams.
• Maintain CRM and track referrals, pipeline, and territory performance.
