Job summary

UnitedHealthcare is seeking a Field Based HSS Coordinator to join our Integrated Care Model. This role relays member needs, advocates for the best care, and supports services in a home-based office while covering assignments within the Eagle Pass area.

You will travel up to 75% to member homes and facilities, complete assessments, document promptly, and coordinate services. Requires clinical experience or appropriate license; strong MS Office skills; transport access and a valid US driver's

Qualifications

High School Diploma/GED AND 5+ years of experience working with ABD/SSI population within a clinical health care role.
Associate's degree with 3+ years of clinical experience.
Unrestricted LPN/LVN license in TX with 3+ years of clinical experience.
CHW certification with 3+ years of clinical experience.
Intermediate level of proficiency with Microsoft Office, including Word, Excel, and Outlook.
High speed internet in place of residence.
Access to reliable transportation and valid US driver's license with travel up to 75%.

Responsibilities

Complete face-to-face assessments in members' homes to address unmet needs.
Travel up to 75% of the time, including travel up to 100 miles from the assigned work location, to meet business needs.
Meet documentation and turnaround times based on state contractual requirements.
Time management and organizational skills.
Coordinating members' services.
Ability to create, edit, save and send documents, spreadsheets and emails.
Reside within commutable distance of assigned duties.
Works on Service Plan.
